The Transition to Adulthood Council (TAC) at OHSU is seeking two healthcare providers to participate in a story-sharing and panel event. 

TAC is a community advisory board through which young adults with disabilities provide feedback and develop training for clinical programs related to the transition from pediatric to adult healthcare.

The TAC is looking for two volunteers to participate in their second annual provider panel, which is organized around the concept that power can be shared through sharing stories. The panel encourages an exchange of knowledge that isn't top down, but is instead collaborative and rooted in personal narrative. 

Based on already confirmed participants, there is a slight preference to for those who work outside of primary care and hold positions other than nurse practitioner.
